LOG INTO THE SBHC DATA HUB
- Using your email address and password, log into the SBHC Data Hub.
CREATE A NEW CLIENT RECORD
- In the menu on the left side of the screen, click Clients to expand the menu.
- Click Records in the menu.
- Click the gray + Add New button
- Complete the following fields:
- Group
- Form (this is the survey)
- Language
- Invitation Expiry
- Client
- Search for an existing client or click + to add a new client profile.
- Email and/or Phone Number
- Click SAVE.
A link to complete the survey will be sent to the email address and/or phone number provided.
The client will need to click on the link to complete the survey.
