New Mexico Healthy Transitions (NMHT) is a five-year project (Oct 2014-Sept 2019) funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) through a grant to the New Mexico Children, Youth, and Families Department (CYFD). NMHT aims to engage youth ages 16-26 years old who are at risk of substance use and mental health challenges and help them access behavioral health and other support services. As the evaluation partner, Apex provides evaluation design, data collection, data analysis, and reporting services. In a related project, Apex provided technical expertise to design, conduct, and analyze the Reconnecting Youth Survey (RYS) with youth and young adults in Santa Fe who are not in school or work.
